On 13 fear is real help???!?

I don't really understand the shows. They can't actually KILL people so what actually happens to them? Like in a lesson in terror with he swinging pendulum, why would it show blood on the door splattered, if they can't actually have it cut him? And since they know they can't actually be killed, then why are they scared anyways?
